I am the parent or guardian of the abovementioned minor and I confirm that they are permitted to train at The Basement LDN Gym + Studio without the supervision of an adult. I confirm that my child will alert a member of staff upon each visit to the Club. My child will arrange a health and safety induction with the Club before using any equipment that they do not know how to use and will in all respects conduct themselves in a safe and sensible manner while using the Club's facilities, including in adherance with the Terms & Conditions and Club Rules.
